This twice-monthly community of practice will support primary care providers in offering gender-affirming hormone therapy (GAHT) to their adult* patients.

This community of practice depends on peer-to-peer learning. Each meeting will include opportunities for general check-ins and case-based discussion on various topics. Participants are encouraged to submit cases in advance of each meeting.

Providers who are interested in prescribing GAHT but have not yet begun are encouraged to attend the GAHT Learning Collaborative on December 7th prior to joining this community of practice. Click here to learn more.

*Course content will be focused on care of the individual who has reached a developmental stage of Tanner 4/5.
